  3. COLORADO STATUTES 15-18-104

  6.         DECLARATION AS TO MEDICAL OR SURGICAL TREATMENT

  8. I Scott Baker, being of sound mind and at least eighteen years of
  9. age, direct that my life shall not be artificially prolonged under
  10. the  circumstances set forth below and hereby declare that: 
  11.  
  12. 1. If at any time my attending physician and one other physician
  13. certify in writing that: 
  14.  
  15.  
  16.     a. I have an injury, disease, or illness which is not curable
  17. or reversible and which, in their judgment, is a terminal
  18. condition; and 
  19.  
  20.  
  21.     b. For a period of forty-eight consecutive hours or more, I
  22. have been unconscious, comatose, or otherwise incompetent so as to
  23. be unable to make or communicate responsive decisions concerning
  24. my person; then, 
  25.  
  26.  
  27.     I direct that life-sustaining procedures shall be withdrawn
  28. and withheld, it being understood that life-sustaining procedures
  29. shall not include any medical procedure or intervention for
  30. nourishment or considered necessary by the attending physician to
  31. provide comfort or alleviate pain.
